The best thing to do is call your insuranve carrier and give them your member number - ask them for specific criteria required for plastics. They are the only ones who can tell you. We can tell you what our insurance required, but it very likely could be different. Also some plans have total exclusions for anything related to plastic surgery. I have United Healthcare and my plan has a total exclusion regardless of the reason for anything related to plastics.
There are others on here with United Healthcare plans and they have had their TT's paid for. So it really depends on your individual plan.
Also, if you are trying to get your ducks in a row - you need to verify with your plan anyway to avoid any errors. I certainly wouldn't take the information from this forum as 'gospel' for your plan.
