First of all, it depends on your employer's plan. Some have a total exclusion of anything cosmetic and plastic surgery even for massive weight loss is considered cosmetic. I'd call your insurance company and ask them what their policy on plastics after WLS. If they say it depends on your situation, then I'd go and start documenting your issues. Rashes, chronic pain, mobility problems related to the excess skin. These things have to be in place and then you can find a plastic surgeon who would be willing to submit the paperwork. Some insurance companies have requirements that must be met as far as how low your excess skin hangs and various other things.
