Sample letter of med necessity for panni??

Does anyone have a copy of the letter of medical necessity written by their pcp for a panni that they would share with me? I'm in the treatment phase, but since I'm an anal retentive, micromanager (lol), I'd like to take a sample for my pcp to review before writing mine.  I'd really appreciate any help you all can give me.

There is a website that I bookmarked a few months ago when doing research on plastics after WLS. There are quite a few letters there that I'm sure you would be able to use.

Hi Emmie,

Just today I recvd a reversal of a denial (YAY!!!!!!) for a panni and abdominoplasty ( for rashes)from my ins co. My appeals rep praised my "appeals packet". The packet included dr's letters, reciepts for visits and rx's and a 2 page letter from me explaining why this surgery was mediacally necessary for me as well as all the methods I had tried  to improve my existing condition. The Derm. also gave rave reviews about the letter from my PCP.  I have copies at work and would be happy to send them to you tomorrow.  I must tell you that my experience with the ins co's (2) has convinced me that not only do you need a letter from your PCP but definetly one from a "specialist" aka derm or ortho or etc.

I have never pm'd anyone on this site so just let me know how I can get you a copy of the 2 letters that worked for me!
