Mens boxer shorts are loose fitting and were comfortable for me to wear while I had drains, no elastic anywhere near the drains & they did not rub on the drains.

I had 2 drains (1 on each hip) from my LBl and was able to wear underwear right over the drain sits.  However, i did not have any discomfort from my drains at all although I know many others have said the drains were the worst part of recovery.  If underwear are bothersome you could just wear long nightgowns, robes whatever sans underwear until the drains are pulled.  how your recovery goes quickly!

Aloha-
I think I was the queen of drains- I had 7 for my circumferential belt lipectomy/ breast lift/ breast augmentation- one in each breast, two from the back hip area that exited from the front hips, two from the lower front abdomen, and one from the pubis.
I wore a slightly baggy pair of "boy cut" briefs that I had worn "on the way down" while losing the weight as well as boxer type shorts with elastic waist- my drains (Jackson Pratts) had bulbs that collected the serum, all were contained within the fanny pack that held the OnQ Pain Blocker (I wore it on the front) or clipped to the belt that held the OnQ Pain Blocker ( best part of my pain relief from my surgery, totally worth any extra expense- mine was covered by insurance).
The drains started to be removed on Day 2, all were out by Day 12 post op- while inconvenient, I had excellent results, never wore compression garments and never developed swelling or edema, even after hard workouts ( I was given the OK to go back to the gym for stretching at 12 days, and weights/cardio at 21 days).
You find a way to make it work! Congrats and continued great healing! XoLori

LOL, i just got my drains out 2 weeks ago.  I just wore pull up pants, like jogging pants.  I wore 2 pair of underware, i would put one pair on, pin my drains to it, put the other pair on to hold them in place, then put my pants or capri's on.  I also slept with 2 pair of undies on too, so that drain wouldnt move too much.  One time i did just wear one pair, my tube came unhooked from the bulb, yuck what a mess! Oh, and i wore my old undies too that are about a size or so too big

Let see, I had 3 in the mons, 1 from each side of my groin, 1 from each arm, 1 from my left breast total of 8 - then 2 more behind my ears with the facelift.  I had cotton 2 snap mens boxers that I wore over my compression garment.  The drains came through the crotcheless opening for the mons & groin.  I clamped them to the top of the boxers or my terrycloth pants.  By the time I went to the hotel, I only had the 3 mons left.  I flew home with 1 mons drain that I hid in a small fanny pack.
