hospital will not submit to insurance

I am having lbl on Dec 9 and the hospital requires you to pay up front.  I ask if they will submit it to my ins.  after ( like the Dr. said) and they said no,  insurance will not pay unless skin hangs to your knees..  I thought the Dr said if you have rashes and  have documentation they would pay for at least the tt part...  I just had my arms done and  they said insurance would not pay that,  I did not question it then but I am questioning this as I heard it would pay part and you pick up the rest..  I had called medicare and my ins. and they do not pre approve but said after it depends on weather the Dr thought it med. needed... such as rash and etc...I am a little upset that they say they wont even try...  has anyone else run into this?

Medicare will not do predeterminations, and hospitals would generally make you pay up front cosmetic surgery fee schedule. If you did fee for service with your surgeon, you cannot submit to insurance for facility charges. If you were done as an insurance case, you may ask the hospital to file and reimburse the difference if they ended up paying. You will still have to front the money for surgery however.

As to your arms, brachioplasty is rarely considered medically necessary and not normally eligible for coevrage of any sort (particularly with medicare). Again, if you've contracted fee for service you waive any ability to file with your insurer.

thank you for your reply. It was my understanding ins. or medicare would not pay for the arms but the dr  said they some times pay for a basic tt if you have the rash and other problems.  you have to pay up front and then submit after surgery.. they told me what it would cost for the surgery and what the hospital charge is for one night stay.  I just did not see how they could refuse to even try to submit it...  I have heard of others getting partial pay and they make up the difference for the total  LBL     I guess all hospitals and Dr are not the same... I thought Cleveland Clinic was a good one,  I guess as long as they get their money  (up front)  they do not care if you get any back...
