Hi there and congrats on your weight loss and upcoming plastics!
I had tt/ lbl and brachio then breast lift w/ thoracoplasty and full 2-dimensional thigh lift. IMHO I would not want to do the tummy and thighs together even if the surgeon would. Having had them done separately I could see the potential for mobility issues. The thighs were my roughest surgery and combining that with the tummy seems like unneccesary torture. lol! I think having the tummy done first before the breasts because you will have a more realistic view of what you actual body shape is. Not to mention, it doesn't seem like a good idea to be having the skin tugged down for the tt/lbl after you have had your breasts put where they belong.
As far as whether the thigh lift is worth it is debatable. While I was recovering from the FTL I regretted having it done. I don't mind the scars but it has taken a while to get flexability/mobility back. I am 4 months post op. It is nice not to have the loose skin on my thighs and now I am glad I had it done. But the difference post op for me wasn't that noticeble to anyone except myself and my husband. I imagine if you have lots of loose skin, then the difference may be more dramatic. I mostly had upper/inner thigh loosness and wrinkling. The LBL took care of most of the outer thigh looseness. You will have to weigh the pros and cons for yourself.
Labial splaying is when too much skin of the upper thigh (in the groin area) is removed and it causes the labia to be "pulled" to an open position anytime the legs are parted and in severe cases even when the legs are not. Not a good thing to be dealing with. In my understanding, once this happens, there is little that can be done to correct it and it is quite debilitating and uncomfortable. That is why it is very important to make sure your surgeon has LOTS of experience in performing TL and speak to his patients who have had it done. Not trying to scare you, just want you to be informed.
I wish you the very best in your decision and in your plastics journey. God bless.

What I had done and when is in my siggy. My ps will not do lbl and inner thighs together. Both of my surgeries were around 7 hours. Trying to do too much at once is a lot for one surgeon to do on his own.
The lbl is complicated surgical procedure and will help address issues with the outer thighs. Inner thighs, well, you want to see where everything else settles before having a ps guess where they might end up. The lbl you have to be flipped during surgery, for inner thighs your legs are spread and your ps is sort of working upside down and it can be quite tedious.
Breasts and thighs together (my second surgery) combined two difficult procedures but to add a third to that... not a wise move for a single surgeon. If there are multiple surgeons involved, then you want to be absolutely clear on how long under anesthesia you'll be.
