Which one first?
Well, it depends on what you want done. If you want a lot of things done, I generally start with the trunk (i.e. tt or LBL) and breasts first and then do arms and legs plus add breast implants if the patient wanted that at Stage II..
Part of the reason is that touchups from the bigger surgery can then be done at Stage II, which is generally a smaller surgery with less downtime and hassle.
I also find it easier to get the bra rolls after a good BL, and to fix the inner thighs after the other parts of the leg have been improved by the TT or LBL.
The one rule that applies to every case is: everyone's body is different and everyone has different priorities.

I was told, for the same reasons Dr L said, to get my TT done first (i'll need an anchor cut). That's fine with me, because my stomach bothers me so much that I can't stand it. My arms aren't great and neither are my breasts, but I can handle those if I have to or when I inevitably run out of $. My stomach I can't handle, at all. It has to go.

I kinda did it backwards and did my thighs first, then the LBL. It seems to have worked ok. I haven't been to happy with the thighs but they are much better than they were before PS. The doctor you choose will be able to tell you where to start. That is if you have a good one. My first one wasn't. I think a good doctor makes all the difference.
