"medical necessity" arm lift requirements?

Hi..
I am 2 1/2 years out and would love to have an arm lift.  I know all insurance/policies are different, but can those of you who have had their arm lifts covered by insurance, please tell me what conditions you had to have/prove?
I have having trouble finding anything out from our insurance company. I am having some tingling/soreness on my one arm and before I see a doctor, I would like to have some info....

My insurance covered the arm lift. I don't know why. That is the only thing they covered for some reason. You should make an appointment to have a consultation with a plastic surgeon. They will know what you need to do to get approved.

Just to give you an idea how insane insurance companies can be--- I had suppurative weeping sores under both arms for years, letters from three doctors, photos of everything, and THIS armlift STILL wasn't covered, and I lost again in the appeal:

My insurance, Blue Cross, covered my arm lift 5 years ago because I was getting open sores in my arm pits and yeast infections.  They sent out a letter just 1 month later to all their plastic surgeons saying that under no cir****tances would they be coving arm or thigh lifts any longer, no matter how many doctors sent in letters.  That may have changed since then.  My PS said I am only 1 of 2 patients he has ever had the insurance cover the arm lift for.  I had letters from my PCP, WLS and PS as well as about 50 photos of the open sores and infections.  I also made copies of every prescription my PCP gave me for creams to heal the sores/infections.

  I saw one plastic surgeon, and all he said is that they have never had an insurance company cover it.  Period.
I think it's worth a shot, but thought it would be better if I had an idea of the "hot buttons".....I know it's not common, but I did see that a few were covered, and thought I'd take advantage of this board and people's experiences.
