Do you know the exact coverage requirements that you insurance has written in your policy? What did they say in the denial? It would help if you posted this information. Details can be very important.

What was the specific reason for the denial and what is the language in the policy.  Those two things will determine how to approach an appeal.

in my police it is not a covered benefit and on the denial it say.  BCBS determined that this service is a health benefit plan exclusion.

did it deny a tt or a panniculectomy?  They are different and generally ins only covers a panni removal.

in your policy does it say it covers plastic surgery to correct a disfigurement due to a disease?  if so then you can appeal.  Some policies cover zero plastics unless it is for cancer, or tragic accidents.  Mine covers nothing.  I have to get a loan to pay for it.  I am going to apply for the loan with the surgeon just so I can put my tax return on it, then I will get a 401 k loan to pay off the medical loan.  I am doing it that way so my 401 k loan will be smaller. (you cannot put lump sums on a 401 k loan- it is either paid in installments or at once.)
