I used to work for Banner in Az and I have to say they had the best insurance I have had ever. I'm pretty sure you'll be able to get plastics through them. They paid for my breast reduction with an overnight stay and I just had to pay my copay of $125 at the time.

I don't work for Banner but my husband does. I have Banner Choice Plus and looked into what if anything they cover regarding plastics. According to the website, they will not cover plastics except for in the case of congenital deformitiy or reconstruction as in the case of breast cancer. Since I did not have any documented rashes and back problems, I figured it would be a waste of my time to even try to get it covered ny Banner. With that said, my husband said that he believes that the plans may differ with the state you live in. Since you work for Banner in Alaska it may be possible for you to get it covered. Call the benefits dept and ask specifically what, if anything they will cover and what kind of documentation they require to get approved. Hope this helps.
