Mine isn't paying for anything.
What I found out that it isn't a matter of "will insurance pay for it or not." But rather, "how is my policy written?" Whether or not insurance will allow it depends on how it's written in the policy your company holds.
My insurance company, said in the letter back to me and the surgeon that they don't deny I need it, it's just that with my company's policy it's specifically written that they do not cover plastics other than for anything related to reconstruction, such as breast cancer.

Policies vary greatly. In general, if they do cover plastics, you have to prove that it is medically necessary (criterion determined by ins), you've documented the problems with your PCP, and you've tried 'traditional treatment.
It is best to contact your ins directly. Find out their policy and start documenting to the exact language of the policy.
