Just like Mack Momma said, you basically have to have medical issues from the excess skin to get covered, or in the case of breast they usuallyw ant 500cc of tissue removed from each side. For reference, a can of soda is about 320cc.

As others mentioned but to expand, your insurance company is looking for medical necessity. To start with, don't call it "plastics" as the connotation is cosmetic and you want all reference to medical necessitiy. I know what you mean when you say platics but you will want pictures if it helps tell how bad the rash or medical issue is, see your PCP monthly and ensure he gives you a pescriptin for the condition, notes the condition in your medical record and get the pescriptin filled keeping the receipts for proof if needed. Do this for three months so show a trend.
