I can tell you that you got misinformation from who ever you spoke to, which is actually not uncommon when you call on the phone.  Do not proceed without that in writing if you want to file insurance, They are unlikely to approve retroactively. You absolutely can get a predetermination for panniculectomy and breast reduction from federal BCBS.

The ONLY carrier whom will not do predetermination that is traditional Medicare and some of the Medicare+ programs.

I have federal BC/BS.  They paid for my panniculectomy - BUT it wasn't pre-authorized either.  They denied the claim twice before they paid it.  All depends on the documentation the surgeon sends in.  The surgeon must be very familiar with BCBS (and Federal) so they know all the guidelines and how to bill it.

HW-218/SW-208/CW-126/ Lowest Weight-121/Goal-125 - hit 8/23/09/Height-5'3"

Regain 30 lbs from 2012 to 2016 - got back on track and lost it.  Took 8 months. 
90+/- pounds lost      
BMI - 24 or so
Starting BMI between 35 and 40ish?

Thanks for the info Jody!  My surgeon's office was amazing when it came to filling out paperwork for my WLS.  They made the entire process almost effortless.  I hope to have the same luck with a plastic surgeon when the time comes.    Thanks again!
