UT-BCBSTX-Insurance Coverage

I currently have blue cross blue shield of Texas that the entire UT system has.  In the benefits booklet it say that plastic surgery is not covered except for medical necessity but then it goes on to only list the few things it will cover, like a mastectomy and reconstruction and childhood reconstructive things.

My question is do you think I have any hope of maybe getting a breast reduction or pannilectamy covered under an appeal? 
Also should I be having my doctor make certain notes in my charts like my back pain, or the scares from my breast being so heavy?  I have lost 80pounds with another 60 to go and I want to start making the right steps to ensure that I can get surgery in a year.

I have BCBS of New Jersey and I was told under no cir****tance will they pay for reconstructive surgery.  Actually this was enclosed with my acceptance letter for WLS.  Anyway, my doctor submitted photos of my belly and breast and BCBS paid for the tt.  If you were denied I think you should appeal, it can't hurt.  Good Luck!
