Can you believe it cost $35 dollars to ship some papers to Mexico??? Crazy....Hope everything is approved and that I don't need to send him anymore paperwork...lol

Oh MY!! Can they be faxed??? I am waiting until November to do mine. I have no idea why, but with the unstable job market, I don't want to give them any reason to get rid of me before hand!!

Dr. S has a fax line, however, it is not a devoted fax line so you have to wait for the receptionist to answer and give you the tone. I don't have a fax at home, so I'd have to send it from Kinkos.  I decided it might be better to send the actual form for him to sign.
I have another form to send for my short term disability now.  I'll do that on Monday.

Be careful waiting too long to turn in your FMLA paperwork.   I know with my employer, I have to have it turned in and approved 45 days before it starts.

How did you send them?  I know you can send stuff by USPS way cheaper!  Did you overnight it?

I sent mine via USPS and it cost $26.00.  He returned them via DHL.  I checked on the prices for FedEx and DHL and they were both more expensive.  The good thing about DHL is that they will come to your house to pick up the envelope.

They were overnighted and the only other price she gave me was with USPS and it was $50 and they wouldn't get there for four days....He told me to use Fed Ex and I didn't question him... Oh well, what's that on top of thousands I'm fixing to spend...hehe

I did it FedEx and it got there within 24 hours. I sent it on a Monday evening at 6pm and was told it would get there by 6pm next night. Dr S had them signed and they were delived to my work by Friday morning. He is quick to sign and get them back. I was very impressed. Most US doctors take forever for everything :-). Mine also came back DHL. I paid 36 for mine. I figure it is worth it to get them to him and back so quickly. My employer requires 30 leahway.

FMLA is specific to each employer.  My facility didn't have an issue with approving my FMLA - it shouldn't matter if you're having your surgery in the states or in a foreign country.
