reconstruction of breasts with own tissue

I am breast cancer survivor 10 years post op never had reconstruction. As I am losing weight I know that I will want  surgery. I am also a BRCA patient and have been thinking about prophylatic mastectomy of the other breast. Figure it would be easier to have both breasts done at same time. Also know that breast reconstruction should be covered.
My question comes from the fact that most plastic surgery is not covered. Could excess abdominal tissue be used and get a TT at the same time? Anyone know if it is possible and reasonable practice

By the way not yet ready for surgery another 70 to go. Just starting my reasearch

There is information about Tram Flap reconstruction on the ACS website.  I think this is the procedure to which you are referring.

Most larger WLS patients will not be candiates for using you abdominal tissue for breast reconstruction as the tissue is just too lax and the circulation kind of dodgy. Unless you have minimal pannus or skin laxity, I steer the WLS patients towards implant/expanders. For bilateral reconstructions, the argument is even stronger in favor of implant reconstruction IMO

Thanks all for the information to get me started. I was thinking more diep than tram. I appreciate the comments about wls size etc. I would like to know when to see a plastic surgeon. About 15 pounds away from goal with weight stabilization or at goal with weight stabilization. Is it too early to begin looking for someone with both breast and WLS reconstruction.

While a DIEP can be performed in massive weight loss patients, I think most surgeons would consider it a relative contraindication. IF you're interested in an authority on this type of surgery in your area, Dr. Josh Levine in Manhattan does a lot of DIEP's and has written some about this issue in weight loss patients.
