ICD9 codes for TT and Breast Lift/Aug ?

Does anyone know the ICD-9 codes for these?

these are the codes that were listed on my surgery paperwork that my friend gave me who is a coder at the hospital where I had my surgery done.

Breast aug and lift
mastepexy -  cpt code 19318  Icd-9  856 ( this is the lift)
augmentation-  cpt code 19324    Icd-9  8550 ( w/o implants)
with implants  19325     8553U & 8554B

The insertion of the implants     19340    8553U & 8553B

Excess skin removed 15830, 15847 for abdominoplasty ( tummy tuck)
00802 anesthia of tt   icd-9  86.83 for all for size reduction, plastic operation

lipsuction 15877   83.65 & 728.84

It seems I will have to fill out the Doctor's portion of Claim form for short term disability.  Does anyone know if we put these ICD-9 codes in the section under ICD-9 disease code or leave that blank (or put something else in that section) and just put it later on under ICD-9 section pertaining to Date and Type of Surgery/Procedure performed... I am clueless to how to fill it out and don't want to invalidate it.

I'm not at work so i can't look up the codes but the 856, 855.0 and 855.4 don't sound right - those are icd 9 codes for an injury and I don't see those these would work with a breast lift.  The 728.84 code sounds OK - it's for a muscle type of condition.

85.50 Augmentation mammoplasty, not otherwise specified.   85.51 Unilateral injection into breast for augmentation.  85.52 Bilateral injection into breast for augmentation.  85.53 Unilateral breast implant .   85.54 Bilateral breast implant.  85.6 Mastopexy .  Correction did not have the period in the icd9 codes , the above cpt codes are correct.
