First you have to establish medical necessity.  This generally means being treated for rashes and such.  And generally means having established a history of this sort of illness.  You will need to see how your insurance defines medical necessity to determine if your issues fit the criteria.

Take photos of the rashes & document issues with your PCP, OBGYN or Bariatric Doctor.

Depends upon the insurance company too.

I have a hernia repair and rashes pending approval for repair.

call your ins. company back and request their medical necessity guidelines in writing... then comply (if you do) with those guidelines and have it documented by multiple phyisicians. Be prepared that some insurance comanpanies dont care about the amount of excess skin... it's what medically documented, treated and hospitalized problems this skin is causing.

My ins company wanted 6 months documented antibiotic resistant rashes.... No way to comply with that since my rashes were yeast and yeast is not treated with antibiotics.

All of the above advice is on point. Everything must be documented. My insurance company initially denied me until my PCP sent documentation of the skin rashes yeast infections and pictures.
