Letters for insurace...Please....

I was just denied from my insurace co.(UHC) I want a panni removal. I need to appeal but I need a letter for medical necessity. Iknow I saw them some where on the board but can't for the life of me find them...Can someone help me?

There's an example of a letter DrL provided within this thread. You would just need to modify it to fit your situation.
