I have BCBSM PPO. The requirement is that you have to be 1 year out from surgery and that you have to lose a minimum of 100 pounds. As long as you meet the criteria, they will cover it. They did do a pre determination to make sure, because BCBS does not just let you know you are approved. As long as you meet the criteria-which I did. It also helped me that I had documented rashes. However, she said that it was not necessary.

My insurance also paid 100% of my TT. I have Cigna and I had to have documented rashes (at least 3 doctor's visits over 3 months), documentation that meds didn't help rash, and pictures from the bottom of my boobs to mid thigh (good stuff covered up). I jumped through the hoops and they paid. Check with your insurance company to see what their guidelines are.

WOW - a WHOLE TT??????? WOW! NICE!!! I am so happy for you!!!!!!! Congrats thats is simply awesome. I wish I could have had that.. then I could afford upper body but no go for me.. took me fighting tooth and nail for panni removal and now I find out I still owe 20% since we dn't have an option for a tier one level doctor (so dumb).
