Issue with Nate need advice

Ok Ladies, I am having an issue. Nate has as had a small touch of acid reflux since he was 8 weeks told. For the last past month Nate has had 1 tablespoon of rice in his 6 oz bottle of formula per drs orders. Now for the last 3 weeks he is eating less and less. The dr said we could feed him by spoon he loves it but then refuses his bottle so i all together just have been trying to get him to feed by bottle. Well the last 2 weeks he has got to where he has not just spit up but throws up a least and ozs are so but he is gaining weight. The thing is he seems very gassy so i give him gas drops seems very unhappy crys alot more then he has ever in his life. Now he had been sick congested and was told he was teething but this isnt like him. He wont even take longer then 30 mins naps. He will be 5 months the 25th and as of last week he weighted 13 pounds and 24 1/2 inches but he weight 5 lbs 1 oz when he left hospital. What am I doing wrong? I know you all are not drs but what is there I can do for this child. I take him to the dr tomorrow but I am not good at speaking like i am not good at typing. I need help on ideas of how to go about explaining to this dr i feel something is wrong regardless if he isnt losing weight. i mean the pour kid wont even lay on his belly he acts like it hurts him. Any ideas would be greatly appericated. I am just scared something is really wrong with my little man. (((HUGS))) I am sad that little man Nate has acid reflux. I hope it gets better. I know since Hunter has started to do teething, he is less hungry. But sleeps longer with rice cereal in his bedtime bottle. He seems to be doing alright. -Jamie & Hunter P.S. been EXTRA busy every night with my other house, so for a few weeks I will be hard to find!!
