When should i see the surgeon?
Personally, I would have made it on the 29th about 2 minutes after walking out of the psych eval... but that's just me. :-)
You're not pushing yourself on your surgeon, you're being your own advocate. You're hiring him, he should be very welcoming of your business. If he's not, that might be a warning sign to look for a different surgeon, because he will be "in your life" for, well, the rest of your life. You need to make sure he's someone you're VERY comfortable with.

My insurance journey was slow and laborious. I learned to get copies of all reports and faxed them to the right people myself. I called my surgeon's insurance coordinator regularly, as well as the person in charge of my case at Blue Cross.
Ask a lot of questions, get a full list of all insurance requirements so there are no surprises, and be sure that you click wi your doctor and trust him implicitly!
