Update

I called Dr. Fangs office after I was told that I needed the co-payment. Dr Fang moved my surgery to December 29th to see if I can come up with the co-pay. So wish me luck I am going to do what ever I need to.

oh g ood now get to work and find a solution. it is there somewhere. ...i dont mean to sound glib but the answers are always there...maybe something that can be sold, maybe a loan, or work extra hours . i know there has to be an answer. You wouldnt be given this second date if the rest of the answer wasnt there. ...I will be praying for you to be alert and on your toes to find it.

I thought about 401 they can help with about 500.00 so part done.

Do you have a 401K retirement plan?  Some will let you take the money from that as a loan or hardship for medical costs.

I know it's tough economic times and something one doesn't want to have to do but. . . . I know  of people who have taken out credit cards and put the complete cost of a RNY on it. I was a self pay and tried to look at it as buying a new car. And I think this "new car" is much more important for me to have than any shiny piece of metal.

Sounds like the copay is money you would have to come up with at some time so just figure out a way to do it now even if half of it has to be on credit. You'll never regret it.
