I see you posted this back in October...  Have you had your appt yet? 
If not, here's how it worked when I went thru it.
I was going thru their program for a while before my insurance changed. I had to get the referral from my Dr to their bariatric team.  After that I got a call from the Marylou Greenwood, the head RN for the program. She got me enrolled in their seminar. 
They will need 6 months of documented weight loss attempts (either through Weigh****chers or Jenny Craig) and a food and excersize journal. I'd get those started now, it'll save the hassle later.
They also wanted me to join their "Freedom From Diets" class.  It is a 5-6 week class and you have to pay for that.  I'd do it asap and get it out of the way also. 
After that, they'll schedule you to see their nutritionist/dietician and physical therapist to go over your food and excersize logs, and from what I've heard, they'll usually go over it tell you what to change and send you on your way to do it again for another 6-8 weeks. You'll meet with them again and if they see that it's acceptable, they will approve you to meet with the surgeon to see if he thinks you are ready for surgery. 
I only made it as far as scheduling the appt with the nutritionist and physical therapist. I never actually got to meet with them.  They book their appts out so far and in my situation they had to keep rescheduling.  They wouldnt have been satisfied as I rarely wrote in my food journal.

I don't know of any off hand, but I hope you find one! I travel an hour and a half to get to mine in Portland (that's when I can make it, It's been a LONG time), It's a bit of a bummer to have it so far out, but the people are pretty great so I hope I'll make it back for another one soon!

Hi! Saw your post on the other board! I love the coast, but yeah, it'd be nice if they were a bit warmer! Although somedays we run to the coast to escape hot weather here so it's kind of a nice refresher! lol. Not to mention it's better weather there for our kite flying!

I didn't even know there was an Oregon forum. I saw a question about it on another forum.
 Glad I found it.

I had the band for two years and 4 months but had it removed and revised to a sleeve in April of 09.
so I am coming up on two years with the sleeve too. WLS has  been the very best thing I have ever done in my life.

I love Oregon but I would like it if summers on the coast were warmer. I do visit friends in Portland a lot in the summer to get away from the windy coast.

Hope to be a regular here now that I found the Oregon board!

I'll e-mail you. My mom grew up just north of there in Creswell. I grew up on the North side of Eugene in a little community (at the time 800+ population.). I'm dealing with a horrible migrain right now, kids are in bed, and that's were I'm heading. I'll put together some links and some general info (as well as my opinion on some nice area's you might check into) in the morning and e-mail them to you.

What an you tell me about the city of Cottage Grove? It LOOKS amazing, on paper!

It is about 20 miles south of Eugene in Southwest Oregon.

4 seasons is NO PROBLEM. LOL. We can do that.

Western Oregon can be pricier but it depends on where you go. Problem with Eastern Oregon is not to many nice/decent sized towns for employement. Some counties in Eastern Oregon have around 30+% unemployement right now.
