Work for State of Oregon with Short Term Disability?

Has anyone who works for the State of Oregon that has had surgery used the short term disability that state employees get to cover the recovery time after WLS? I am trying to find out if you had the froms filled out before surgery and submitted or did you have to wait until you were home from surgery and submit the forms? It would be so much easier if after you have the surgery date you submit the forms and not have to worry about that when recovering. Any help on this would be appreciated.

It's been so long I can't remember what order I did everything in, but I had no problems using my short term disability 2 years ago, and I even went to Mexico for my surgery. Just contact payroll and they will walk you through what you need to do. It was pretty straightforward and rather easy. Dina

Hi Michelle With our state agency, we have to use up all our leave accruals (SL, PB, VA) before we can start our short/long term disability. Check with your payroll office to see what your departments requires. They also would have the forms to fill out. Nancy

Good point to check on what leaves you have to use up. I had to use up sick leave but I'm pretty sure nothing else. Although I did use all my other leaves, they started paying as soon as the 30 days kicked in regardless of whether I'd used up any leave, so essentially it was as if I was double paid for a few weeks. This was the case with my surgery in 04 and my c-section in 01. Dina
