Got my orientation questionnaire, have a few questions

I'm brand new to all this. My Dr. sent my referral Sept.23 and I got my orientation questionnaire in the mail yesterday. 

Just curious,

Why do they ask about income? Has anyone been denied because of this?

Sleep habits, can you be denied for snoring? or over-tiredness?

I'm excited and nervous at the same time. Really hoping to get through everything and get closer to the one thing that I think can finally help me overcome my struggles with obesity.

The income question is asked because there is some out of pocket expenses...you will need to purchase optifast before surgery which is just over 100 per week and ....and you will be on vitamins for life.  B12, calcium and D3 and a multivitamin.

As for sleeping habits and snoring they are looking to see if you could have sleep apnea.  If they feel they need you to go to a sleep clinic they will send you if need be.

What Laura said.....they want to know you can afford the Opti-fast and post-op vitamin regime.

Is this a clinic specific questionnaire? I have orientation at St Joe's, end of November and I haven't received anything other than my orientation letter.

Mine is specific to Toronto Western
