How do you get your surgery type and date on the top of your posting?

I've searched and searched and can't figure it out?

Medical history under the settings tab! The lil rivet like wheel...thingy in the corner, haha.

Thanks!  I had the info in there but didn't have the Primary box checked off, d'uh!!

I'm having difficulty getting my surgery type and date in the title section of my profile.  I'm going into Medical History, adding the information, checking the primary box and hitting save... but it doesn't seem to save.  As soon as I go back in, all the data is gone.  Can anyone help?
