Medical Bracelet for RNY

I want to order a medical bracelet for myself following my surgery two weeks ago. What is it that our bracelets should say on it? My handout from TWH lists them and where to order, but doesn't really say what should be on it. Thank you.

I ordered one a couple weeks ago from a jewelry store it was 50$ and all I put on mine was 

gastric bypass

roux en y

my emergency contact name and phone number

i had room for more but figured this got the point across lol 

First-name last-name 

gastric bypass

no blinding tube

no NSAIDs/no sugars

dr's phone #

No blind NG tube. (Damn autocorrect)

I got one a month after my surgery and it says on the front:

Roux-En-Y Gastric Bypass in February 2015

On the back it says:

No blind NG tube
Conditions:  Asthma
