No matter where you live, you can and should get a referral to a ps anywhere in Ontario.

Tell your Dr you are insisting on a referral to whichever name it is you would like. Honestly any ps will see you, you don't need a referral, but it will remove the cost of the consult which is about $150.

who was your surgeon? Dr Sullivan told me he would gladly refer me if needed ETA: I see it was Dr Smith.. he should be able to refer you! Call his office
