Contact info for St Joes

Had surgery in March 2012, went for 2 follow up appointments and sort of fell off the earth after that? Experienced different issues due to major changes (including weight loss) in my life and had a bout with addictions....

Am healthy and on a proper path right now and would like to proceed with following up with my clinic which was St-joes....I honestly don't even remember my surgeons name and don't receive mail from the hospital as I've moved a few times since surgery.

Sounds like a mess but please keep opinions to self? Just want info :) 

Is there more then one st joes? Is it Hamilton? 

I am assuming you were at St Joes in Toronto (not Hamilton?) Does Dr. Smith, Dr. Lindsay or Dr. Sullivan ring a bell? You still in Sudbury? Feel free to drop in for our monthly coffee night tonight if you are. Tim Horton's at Lasalle and Barrydowne at 6:00pm/
