Who gives the note for time off work -surgeon or family doctor and when do you get the note?

Orientation Hamilton: July 22, 2013 Ultrasound: December 18, 2013 Bloodwork/ECG: January 29, 2014 Nutritionist/Social Worker/RN: February 12, 2014 Gastroscopy: March 4, 2014 Internist: March 6, 2014 Surgeon: March 13, 2014 (cancelled by clinic) Echocardiogram: March 19, 2014 Rescheduled Surgeon: April 1, 2014 SURGERY: April 14/14

Mine was actually mailed to me from the Ottawa Weight Management Clinic.

I'm in Ottawa too and they sent something in the mail to me for time off purposes.

Ask your centre/surgeon and I'm sure you can get something from them.

TWH gave me the letter in the hospital when I was discharged. They would not do it until I had actually had the surgery.

The surgeon's office filled out my form (my workplace has one) and sent it in.

Referred to Guelph, Dec. 3/12. Orientation: Mar 7/13. NUT/SW/RN Jun 18/13. Nutrition Class Jul 3/13. NUT/SW/RN Aug 19/13. Post-op Nutrition Class Dec 30/13. Approval for surgery from Dr. Jules Foute Nelong Feb 10/14. Surgery Apr 23/14.

my surgeon provided the note and it simply stated time off work for surgery & recovery time and noted that when return to work should be light duties with no heavy lifting for 3-4 weeks after, had no specifics of what the surgery type was.

I work a desk job, but just remember even if you perform clerical functions there are heavy file boxes etc.

prior to surgery 323lbs....4 years post-op maintaining between 108- 114 lb loss.
