is it internist, surgeon or it's a team decision by all the specialists ?

From my experience, the internist decided if I could handle surgery and the discussion of what kind of surgery was between the surgeon and myself.

It is the surgeon. However in Ontario the RNY is considered the gold standard. Unless you can present reasons for another kind of surgery that is what they will do. There is certain criteria for a sleeve.
