Note for time off from Surgeon
Hey Guelphers ...
When do we get the note from the surgeon explaining how much time we need off work? I remember that they don't do it until after the surgery.
Has anyone required that they fill out a couple forms for work's HR department and not just a note? I have to send forms to a 3rd party confidential Health and Wellness company that notifies my HR dept that my time off is kosher.
I'm just wondering if I give the surgeon the forms the day of surgery or give them to the clinic and pick them up at my 2 week appointment???
Also, does anyone call from the clinic or hospital with a confirmation before surgery? I'm getting nervous! I'm having dreams that I show up on Tuesday and nobody knows about me having RNY :) yikes!!

My GP did my paperwork for HR, She was copied on all my appointments by the surgeon.

My surgeon emailed me the letter for my employer well before my day of surgery. All depends on your surgeon

your note simply has to state that you will require (x) amount of time off due to surgery...NO WHERE does it have to state what type of surgery you are having. Nor does your HR dept need to know the reason.
You can pick up a note on official doctor letterhead/notepad prior to your surgery.
I had to drive to the USA for my surgery and therefore I called the Bairiatric unit the day before to make sure I wasn't cancelled.

The NP at my TWH Surgeon appointment gave me a letter before my surgery, but unfortunately my benefit provider needed documents filled out so I had my GP do it after my surgery and I mailed them to the office.
