You've GOT to be kidding!
I actually did not have a family doctor... I had my referral sent in from a walk in clinic. A week later I did get a family doctor (I was on the ontario waiting list) which is good cause I don't think the Guelph clinic would have let me go further with out one.
I would definitely look for a new doctor.. perhaps get a walk in clinic to sent the referral.

hey all
Thanks for the encouragement - I hadn't thought about his delaying as discrimination - it always helps to have an extra mind (or a few hundred) to give different perspectives :)
I've signed up, and it's a new doctor so I can't book an appointment until next week (they aren't pre-booking him).. but when I go in, I'll have all the information on how to do the referral, all the research I've done on it, and what the previous gp put me through, and hopefully leave with a commitment from him to do the referral quickly.
Thanks again everyone! It's great to have such a supportive community to go through this with!

LOL - wouldn't you know it - I get a phone call back from the doctor's office today (have to wonder if he was looking at this forum and realized he was about to get a CPSO complaint against him) :)
He has the form completed and wants to go over it with me before sending it. Only took him a month.. *rolls eyes*
At least it's finally moving forward
