Surgery March 23/2011. Completed three full marathons and two half marathons, two half Ironman distances. Completed my first Full Ironman distance (4 km swim, 180 km bike, 42.2 km (full marathon) run) in Muskoka August 30/2015. Next Ironman Lake Placid July 23/2017!
You must be at goal weight for a year before they look at you (that's OHIP, not the surgeon's decision). If you wanted to go now, without being at goal, then yes, it would be about $8k (I asked my surgeon how much it would be if I wasn't approved). I researched my surgeon, and know a few people who went to her.
I also like the hospital, and have heard many good things about the care there (and I was born there!) I went to my family doctor with her name for the referral and then had the appointment in August (a year at the hospital's goal weight for me...ish) and then the surgeon sent in the request. Other than what I told the surgeon, I personally didn't send in any documentation.
