LOL ... Thanks for doing this, I'm heading to the CIVIC campus for the same on the 3rd (was wondering what they were going to do)

At the civic you go to the location. They weight you take your vitals and then the nurses goes over the white 5 page questionnaire that you filled out before your surgeon appt. Someone comes for blood and maybe and ECG. Then the anesthesiologist comes in and then you are done.
