Northern Ontario Travel Grant Question

 Wow good for you Mary.
Where did you travel from?
Was the 10 nights from when you had your surgery because I understood it that they only paid for 1 night? 
OMG I can't believe how confused I can get these days. Damn Bronchitis. 

Sometimes you just put one foot in front of the other, even if you don't know where you're going.

OOPS...sorry it was (1) night NOT 10

I travelled from Thunder Bay - Toronto

prior to surgery 323lbs....4 years post-op maintaining between 108- 114 lb loss. 

life is AMAZING when you continue on the right path~!.  Use your surgeons gift to the fullest~!

 LOL I was going to say how did you do it share your secret lol. It sounds like they paid you the amount for driving not flying because it's  41 cents per km minus 100km. SSSShhhh we won't correct them. lol

not sure if they made  a mistake or not.

I highly doubt it because they in my original submission I only sent in my boading pass (each way)...dummy me. 

They then sent a letter requesting my actual plane ticketreservations/informaiton showing breakdown of cost.

either way....as I stated I appreciate being compensated for the trip.

Hey there,

Do they accept Toronto Western Bariatric Clinic as a referal institution or does it need to be to a specific DR?  Sorry....been going through the process for a year and no one ever told me about the grant....definitely bringing it for further consults and surgery

 Hi, The referring Dr is whoever sent in your paper work to have WLS. Toronto fills in the lower section of the grant.  You only having to get your original referring Dr to fill their section out once but Toronto and you have to fill your sections out each time.  If you have any more question please feel free to message me. Do you have the travel grant form?

Where are you travelling from? You can only apply for the travel grant if you are from up north. If you do live in one of the districts that are eligible, I am surprised your family doctor never gave you the form. You could likely get one now from your GP and have the hospital go back and complete one for each visit you made. They have to all be no older than 12 months old.
