Hello, I recently joined this site. My surgery date is October 13, 2015 at Cleveland Clinic. When is your surgery date?
Also my pcp referred me to the program and I never had to fill out any kind of application. I just had to complete all of their requirements. It took like 6 months.
Yea your pcp won't put in for the auth. The surgeons office will.
Okay, so. Insurance told me that I didn't need a referral from my primary care doctor and that I could go straight through a surgeon. Problem is, I have to fill out an application before bariatrics even sees me. So, would I make the surgeon do the authorization?
