Medicaid therapist who knows food issues

I am seeking a therapist who 1. Takes medicaid 2. Supports bariatric surgery 3. is experienced with food issues 4. is in the general Columbus area Please give me any recommendations you might have. I would like to begin seeing someone pre-op to establish a relationship so if I run into any issues post-op--I am not suddenly scrambling to find someone good! Thank you!
