Help !!!

My family doctor wants me to type a letter for him to go over so that he can send it to the medicaid for approval,, Im not sure where to start. not sure what all needs to be in the letter,, I know I need the most important info. but what does that contain. anyone have an suggestions for medicaid approval, and what there letter said.

My doctor didnt want me to type anyting up, but he wanted to know everything I ahve tried in the past so many years and then he typed it up on his letterhead. I don't think the Insurance companys can argue with that. I would ask him if he would do that for you. Iwould see it carrying more weith, Good Luck, Richard
