I don't believe Dr. Cook is on this forum at all. There is a Dr. Curry in Cincinnati that regularly posts here. Are you looking for him by chance?
If you are interested in the OSU program I can help you out there, get some information to you and help you get the whole process started!
Just send me and email with your mailing address and I can get an info packet to you. We have info sessions the first Thursday of every month that you can attend as well.
The number to call at OSU is 614-293-2888. There will be a list of options to choose one, choose the surgical one, I believe it's either 2 or 3.

Ok, I am gonna help you as best I can!!
First, get that application filled out for the Bariatric Department. The first thing they want to do is make sure that you are a good candidate for surgery, so fill it in to the best of your ability and include as much information as possible. I think there is a pre postage paid envelope for that or there should be a fax number included.
OSU is contracted with Medicaid/Medicare. We have a nurse who is the insurance specialist who will help with filing your claim and taking care of that. Her name is Kathy Horton. Once you have been approved with the department, they will send you a letter letting you know what the next steps are.
This is not a quick process, so be prepared to go through some hoops. I know it can get tricky with the medical cards, and most of the problems they face with getting patients to surgery are insurance related.
Give them about 2 to 3 weeks to get your application in and looked at. We had over 3000 applicants last year so you can imagine the amount of mail they receive is enormous! Shannon Surber is the program director, and she can also help you with your application. She's great.
So, get that application filled out and in, they will guide you the rest of the way.

Ohio Medicaid covered my surgery. At the time I had to have my PCP get prior authorization from Medicaid...OSU wouldn't even look at my application until I had Medicaid prior authorization. It took a while to convence my PCP that SHE needed to write the letter, but once that happened it was smooth sailing. I have no idea what she wrote, but it was the first time she had written a letter to Medicaid for anything and I was approved in 2 months. At that time I had already been trying to lose weight for over a year.

I really just do the old email thing! So, if you want to send me your email address (click on email me) that would be fine. I email a lot of people. I tried the instant messenger thing and got so overwhelmed I decided that maybe it's better for me to answer one email at a time!
