I have BCBS of Alabama I called insurance they said if I gave everything to doctor they are asking for then it is up to them to re send everything. Audrey, There is an insurance forum here. Someone there might be able to help you. When I was denied, I got a lawyer to help. It costs, but it would cost a lot more if I had to self pay.

In Ohio, some of the Bariatric surgery places have a "self pay" cash option where you can pay as little as $100 a month without interest charges. That might be an option for you. If you actually just go to the surgeon and tell them you are willing to try and pay cash you might be very successful and get a deal as the insurance is bound to pay pennies on the dollar. And the you would get in faster. I know when I wanted to do a course of hypnosis they would not pay. Then on my own I asked the hypnotherapist if he would give me a deal if I paid cash. Suddenly the sessions were $40 an hour instead of $100!
