appeal

I first applied for this surgery last Feb. I was denied. They said they needed to see proof of dieting. So I dieted and then sent that in. I called them the other day and they said I was denied again because it had been over 6 months. I have to start over. Sometime this summer, I found a letter that someone had sent for their appeal. Now of course, I cannot find it. I would like to use parts of it to have sent with the new info. Someone help me find it again please!!!! Ha!!
