I am worried now that I will not get all the information I need to my insurance and to OSU. The packet from OSU said it is my responsibility to make sure everything gets to them. For those of you that went through OSU, did you have your Dr make copies and then you sent the needed documentation to OSU? I just want to make sure I do all of this right.
I meet with the Dietician and the Psycologist on November 2nd. Wish me luck. I started a journal on what I eat everyday, what my blood sugar is and how much weight I lose in a weeks time.
Any suggestions will be appreciated. Thanks to all of you that have responded to my messages in the past. You are all special to me. We all just need to keep smiling and don't give up.

I am not going through OSU as you know, but my suggestion is that you should have your own personal copies in a file at home of everything that is submitted. That way when they lose your papers or claim to not have enough information, you will know what you need to get. If they require you to see certain specialists, make sure you have copies of those reports as well (sleep study, etc.) I am so glad you finally got the ball rolling with this. I know it has been a challenge for you. You'll have a date before you know it. I'll be praying for you.

Julie, I agree with Jaimee - get copies, especially of anything that might be hard to get copies of again. You just never know, anything could happen to paperwork. I made a copy of mine at work (shhh) and delivered them to Dr. Martin's office in person, just to make sure nothing happened between the mailbox and the doctor's office. You never know when you might need them again.

Oh absolutely make your copies and deliver them yourself (either snail mail or fax). I was lucky I just walked upstairs and hand delivered them.
But it is better to be safe that way, things get lost in the mail, lost in fax world etc.
I would go to my Drs office and get the copies, letters, etc in person. Just call ahead and tell them what you need and when can you pick it up. Make a file, like Jaimee suggested. I did, it's pretty thick, but it does come in handy when something has to be resubmitted for what ever reason. Plus you have a record of who has what.
Which dietician are you meeting with? GOOD LUCK! They are all super. Good idea on the documentation. That will really help them get a good assessment as to where you are right now. They prefer at least 3 days worth. I would imagine you are seeing Nancy Weese. She's my bud! My office is in the clinic building right around the corner from hers! The Nutritionists are a great source of information and support there as well.
But, it sounds like you have your duckies forming a nice line!

Upon contacting your insurance, Make sure you have keep notes of everytime you call them and to whom you spoke with. The person who checks your paperwork before it is handed to review. Ask them if they can be your contact person and ask them for the extension. That way you only go through one person. Good luck

Hi Julie! I am going through OSU - will be having lap-band surgery on 12/2 with Dr. Needleman.
My PCP's office was great. I called and requested copies of my files and they had it ready in a folder for me by lunch. It made filling out the application easier as far as weight and diet history too. They had a chart that listed just my weight for every visit I had made over the past 6 years I've been going there.
I made all my own appts except for the nutritionist, gathered all the info, made copies and Fed Ex'd the entire packet to OSU. I got my approval letter back from OSU within 4 days!! I made sure that I had everything they needed.
I did a journal too, but detailed every step I made in the process...who I called, who I talked to, what they said, etc. It has come in handy time after time for insurance and answering questions here on the boards!
