Doctor Letter of Support

Hi everyone, I need to have my PCP send a letter of support to my insurance company. Anyone have any samples of what worked for you? I would like to have a sample to show my PCP. I would appreciate any help that you could give. Kelly C

Kelly, the rough draft of my PCP letter is in my profile if you would like to use it. Eventually I hope to type in the final letter, I just haven't had the time yet.
