choice care questions
Hello, I'm new here and have my first consultation with my surgeon on monday July 7th. I have humana choice care and they have told me that they cover my surgery but i know I still have to prove that I have attempted weight loss in the past. I am worried about being able to document this throughly. Does anyone know how strict choice care is with this? Does anyone have any suggestions on how to document things?

Which hospital, or surgeons group are you seeing? I also have Humana Choice Care, and am going through UC physicians. I had to fill out this packet, and one of the questions was "Which of the following diets have I tried." It then listed what looked like 50 different plans. I personally have only tried 5 different types, but most of those I tried 5 or 6 times each. I have still been approved by the insurance and my surgery is July 14. My surgeon is Dr. Keith Gersin.

I am going to advanced bariatric centers in northern KY. I have a similar packet. My main concern is I don't have any real way to prove I've tried most of the weight loss methods I have tried. I only have two actual perscriptions for weight loss meds. Do you think that is enough? I also listed gyms I have joined and excersice classes I have tried. I hope these are enough.
Aubrey, I really don't think you'll have a problem. Just besure to check off everything you've ever tried. And the few that you have tried multiple times, besure to note next to it. I think that'll help. And those excersize classes should help to. I never had to "prove" what I had been on, so I don't think you'll have a problem. Let me know what goes on.
