COUPLE OF QUESTIONS???
MY SECOND QUESTION IS THEY REQUIER A DIET HISTORY NO A CERTAIN AMOUNT OF TIME THOUGH. I HAVE TRIED ALOT OF DIFFERENT THINGS BUT NOT WITH A DR FOR YEARS SO CAN I GIVE A WRITEN HISTORY OF THE THINGS I HAVE TRIED ON MY OWN?

Your best bet is to call or log on to your insurance web page and find out all the docs that are in network in your area then start researching them. I ask my PCP and then my insurance and then started researching 5 of them several were ruled out right away. You will then find the best doc for you and your insurance.
As for the diet history again call your insurance and ask what do they really mean. Mine wanted something written from my PCP from the past 5 years weight height if I took any diet RX did they want me to do WW etc. I also wrote my own letter with all the OTC things I did also.
Your best bet is to call insurance and ask each policy is different as to what they want.

I suspect that you will need to do a supervised diet and exercise program. I had Aetna and they required physician's office visit notes detailing the counseling and diet/exercise tracking he did during the period. WW and other diet programs are not proof enough. I think the key is "medically/physicain supervised" and most people do not have the physicians notes from their diet attempts. -- Tom
