I had MMO when I had open RNY 5 years ago. All that was required way back then was:
5 year weight history, BMI over 40 or 100 lb over "Ideal Weight" , med ness. If you go to my profile 9/19/2003 you can see the letter I wrote to MM and my time line. After all the paper work was sent it it wasn't long till I got my approval. In the system 9/24/03 approved 10/10/03
What I would do is call MM and find out what YOUR POLICE states. Each policy if different and then follow what they want and you'll get approved.

Hi - I am a year out and when I started my journey - it took almost 7 months from start of process to surgery day. My Med Mut insurance required documentation of a 6 month supervised diet from either my primary physician or as some have done, Weigh****chers documentation. Once that was done and submited for approval it was about 10 days to two weeks. If you have any comorbidities like Diabetes, high BP, etc, the approval is more likely, but I know some of my friends that had no comorbidies were easily approved. Good luck in your process!
