What do you take to your 1st appointment besides your medical records? For the shrink, dietician and sleep study do you do these before your 1st appt with the surgeon (PCP referral) or does the surgeon refer you to the people they use? I'm interested in the procedure for Dr Lane or Dr White. Thanks

You can get in with Dr. Lane without knowing what your insurance needs. But it helps. for your psych exam he has a doc he likes to use and will provide you with his information and a checklist to get everything done. Dr. Lane will also provide you a list of requirements. The bloodwork paperwork, chest x-ray, ekg. and things. More then likely your first appointment will be with Amy and not Dr. Lane. Amy is great and such a HUGE help.

You need to have met all of your insurance requirements and get pre approved before you have your consult with Dr. White.
1. Call your insurance company and find out if the surgery is a covered procedure
2. Find out what all of the requirements are and get specifics. For example, if 6 month supervised diet find out what is acceptable eg. PCP,
2. If yes, contact your HR dept and assure that it has not been excluded for your specific policy.
3. If  not, begin to meet all of your requirements.  Start the 6 month diet plan if required, then contact a psychiatrist for the psych eval if required.  If dietitian visits are required for insurance contact Kristi at Dr. Whites office.
4. When you are close to fulfilling your requirements contact Kristi at Dr. Whites office to go to  seminar.  You can also contact her before to help with requirements.
5.  If you have met all of your requirements at this point Kristi will submit to insurance for approval
6. Once approved Kristi will contact you to schedule appt with Dr. White for consult.
7. If Dr. White accepts you then they will schedule a date and/or send for other tests if they feel they are needed to assure you are healthy enough for surgery.

Somethings that may change this process up a little is:
What is your BMI?  If not sure, tell me your height and weight.
If lower do you have any comorbidities?
What insurance do you have?
You can PM me those answers if you want.

If you get back to me by early afternoon I will be able to respond.  If later it may be a day or so.  I am having surgery this afternoon so may be a bit out of it till evening or tomorrow.

PM me if you have any other questions.  If they are specific to Lane I cannot help you there,

Terisa

Thanks. I'm going to ask my PCP for a referral in case its needed for insurance. Hopefully there is not a long wait to get in to see Dr. Lane.
