Anyone have Dr Choban for surgery?
Hi. I also live in Grove City and Dr. Choban is my surgeon. I had my appointment with her Aug 5. She is very friendly and approachable. Even though you fill out all the paper work when you first applied to the program, I still had to fill out more paper work. A lot of it the same stuff. I hadn't done any of my testing yet, so I am working on that. I have half my Cardiac done. And I did my Psych eval this morning.
So, back on subject, Dr. Choban's office will be a breeze. Make sure you have a list of your meds, surgeries, and insurance stuff if you can't remember it all. And you will finally get to meet Charlene.

No I haven't been to the support group yet. I am new at getting this stuff done too. I have :
1. been to the orientation meeting
2. submitted my health history
3. submitted my $300 administation fee
4. saw Dr. Choban ( I wanted to ask some questions)
5. been to the Cardiac guy (I need an echo and a stress test)
6. had Psych Eval today
7. Echo is set for next week
8. Sleep doctor is set for Oct. 23 (can't get in until then)(I don't need a study just a check up)
9. I need to get a appointment set with the nutritionist
I think that is all that is on my list . After I see the nutritionist, Dr. Choban said is when she writes the letter to your insurance company asking for approval.
I am a little scared about the stress test. But my EKG was normal so I don't expect anything. I want this really bad also and I am afraid something is going to mess it up.

That is how I feel, I want it yesterday!! I am trying to ru**** along but I know it is going to take at least 6 months since my insurance requires the 6 mth doctor supervised diet. I have sleep apnea so I bet I will need a check up too. So much stuff to do before surgery but I guess that will help the time go faster.

I was banded by Dr. Choban on May 28. She is a great doctor and the support staff is unbelievable. I had no problems at all with getting my paperwork off to insurance. You do have to see the hospital nutritionist and you will have to go to a group nutrition class before they send your paperwork to insurance for approval. There is a good size group of us from Mt. Carmel's program that hang out on a different website under this thread, including many of Dr. Choban's patients, so if you want, come on over there and chat with us. Several of us get together in person also.
